If you need to surrender an animal into rescue care, please notify me a.s.a.p. and include all of the following information to the best of your knowledge so we can see how best to help you and your animal. 
As always I invite you to contact me anytime if you have any questions or concerns.
 
Surrendering Animals
If you could please contact me with the following information completely filled out to the best of your knowledge, I will be better able to aid you in either personally being able to take in your animal, or in some cases, immediately placing it for you. 
 
1.  Reason for surrendering animal(s).  Be totally honest.  I am not here to pass judgement, only to find out as much as possible about the animal coming to me so I can better care for it.
 
2.  Age, Size (this includes snout to vent length and snout to tail length for lizards or just total length for snakes), Weight of Animal(s)
 
3.  Temperament:  be sure to be totally honest on how aggressive or "tame" an animal is.  If you know the animal's likes and dislikes, do include all that here. 
 
4.  Current Health Status and Past Health History - tell me about any and all scars, deformities, internal and external health issues that you know of. For History, tell me about any and all problems the animal(s) ever had or prior illnesses it battled during its life.  This helps us more than anything as often times we don't have a clue on the health history of an animal when it comes to us.
 
5.  Past and Current Diet:  the only reason I ask about past diets is to see if there was anything in the past that might attribute to the way an animal is now.  That's not to place blame on you or anything of the sort.  For example, our Sheena (our green iguana) was fed mice in her past (hence the reason for her chunky size and huge muscles).
 
6.  Past and Current housing situation (I ask the past again for the same reason as the diet).  In current housing, include all information as to what type of cage/habitat, what's in it (branches, pools, fake/live fauna, etc), lighting/heating elements, daytime and nighttime gradients, humidity levels, etc.  Note:  some of this only applies to reptiles.
